- Download our Research Packet (Word DOC Format • PDF Format). Fill-in the information that you already know and return it to us. We will contact you with any questions.
- In approximately 30-60 days you will receive a spiral bound research package with our findings.
If you prefer to order by mail or fax, download a Research Packet. After filling out the forms, they can be faxed to (801) 531-6891 or mailed to Lineages, Inc., PO Box 417, Salt Lake City, UT 84110-0417.
